Job # 2622 Provider Relations Manager (Hospice)


ESSENTIAL JOB FUNCTIONS:

SALES
Duties: Generate business for the hospice by creating new and maintaining existing relationships with physicians and other medical professionals, insurance, financial, and legal professionals, senior organizations, appropriate special interest groups, discharge planners, nursing home professionals, and other community-based referral sources. Meet quarterly goals as determined by Executive Director (ED) and/or Regional Director of Sales. Participate in local and government sponsored organizations, making presentations and joining as a member where appropriate, to facilitate a broader knowledge of the hospice’s capabilities. Pursue managed care contracts in collaboration with ED and RDS. Attend sales and operational meetings.

COMMUNITY RELATIONS
Duties: Monitor the community's needs, health care delivery systems and satisfaction levels; recommend services and programs to address the new and changing needs within the service area. Attend special events to increase awareness among consumers and/or referral sources. Participate in community events sponsored by the company.

TRACKING & REPORTING
Duties: Prepare and submit reports as required, including weekly Productivity Trending Report and Account Profile Territory Book. Create sales plans. Monitor referral and utilization information. Report progress versus goals and census development barriers.

PATIENT CARE COORDINATION
Duties: Collaborate with operations and clinical staff to ensure the intake process meets the needs of the patient, family and physician/other referral source.

CONTINUING EDUCATION
Duties: Participate in orientation and ongoing education programs and training.

POLICIES & REGULATIONS
Duties: Comply with company policies including the company’s code of conduct and business ethics. Comply with state and federal regulations.

BUDGETARY / FISCAL
Duties: Assure hospice meets and/or exceeds budgetary revenue goals. Contribute to the efficient utilization of company dollar by maintaining reasonable and approved expenses.

CODE OF CONDUCT & BUSINESS ETHICS
Duties: Must adhere to the Company’s Code of Conduct and Business Ethics policy including the documentation and reporting responsibilities.

CUSTOMER SERVICE
Duties: Work closely with ED to assure exceptional customer service. Provide customer service training as needed.

OTHER
Duties: Contributes to team effort by accomplishing related results as needed.

Physical and Sensory Requirements (With or Without the Aid of Mechanical Devices):
Mobility, reaching, bending, lifting, talking, fingering, sitting, carrying, standing, grasping, fine hand coordination, ability to hear, ability to read and write, and the ability to remain calm under stress.

QUALIFICATIONS:

  • Bachelor's degree in marketing, business or related field preferred.
  • Minimum three years' experience selling healthcare services with demonstrated results.
  • Home care experience preferred.
  • Excellent verbal and written communications skills, including an ability to work closely with a wide variety of people while effectively using problem-solving skills.
  • Computer skills at a level necessary to accomplish this job.
  • Proven skills in working independently; self-motivated and goal-directed.
  • Knowledge of or a willingness to learn home care state, federal and other regulatory requirements.
  • Must be able to relate professionally and positively and work cooperatively with other employees at all levels.
  • Must be capable of maintaining regular attendance.
  • Must be able to relate professionally and positively to corporate staff and to work cooperatively with other employees at all levels.
  • Must meet all local health regulations, pass post-offer drug test, and pass post-employment physical exam if required.

Job # 2602 Director of Clinical Services

GENERAL PURPOSE:
Responsible for planning, developing, implementing and evaluating all patient/family services. Manages patient care staff. Supports the Operational and Provider Relations activities of the organization.

ESSENTIAL JOB FUNCTIONS:

COMPLIANCE Duties: Ensures that all patient and family services offered by the agency comply with federal, state(s) and local regulatory requirements according to the standards established by the State Board of Nursing.

CLINICAL OPERATIONS Duties: Develops, implements and evaluates clinical operations policies and procedures and a system of staff communication that ensures coordinated implementation of plans of treatment and the utilization of necessary services for comprehensive care of the patients that reflects the philosophy and objectives of the company.

MANAGEMENT Duties: Interviews, hires and supervises clinical staff. Evaluates staffing to provide sufficient personnel for delivery of patient/ family services, including on-call, pursuant to company policies and guidelines. Makes recommendations for staffing to the agency manager/executive director.

STAFF DEVELOPMENT Duties: Plans, implements and documents the orientation and ongoing education of the clinical staff. Participates in the orientation and ongoing education of the clinical staff in the hospitals where hospice has contracted for inpatient hospice beds. Participates in the development of personnel policies and position descriptions that delineate qualifications, duties and responsibilities for all levels of clinical staff.

QUALITY ASSURANCE Duties: Maintains ongoing quality assurance activities related to patient and family services including quality improvement (QI) studies and chart audits to ensure the quality of patient/family services.

FINANCIAL MANAGEMENT Duties: Participates in the budgeting for patient-care expenses and monitors expenses related to patient/family services. Collaborates with executive director to maintain salary/wage/benefits goals.

LIAISON Duties: Serves as the clinical liaison with the executive director/ agency manager. Serves as a resource person and liaison for the hospice care team members who do not directly report to the DOCS. Leads the interdisciplinary group meetings and serves as a liaison with the medical director and clinical staff. Serves as a liaison with vendor and providers affiliated with the hospice on a contractual basis. Monitors the quality of services provided by these vendors/agencies.

COMMUNITY ACTIVITY Duties: Participates in state(s) and national hospice organization activities.

OTHER Duties: Contributes to team effort by accomplishing related results as needed. From time to time this position will be expected to perform on-call professional duties.

QUALIFICATIONS:

  • Must be a registered nurse with a current state license.
  • Must meet all state and federal regulatory requirements.
  • Must have current CPR certification.
  • Must have a minimum of 3 years’ clinical management experience.
  • Must have the ability to direct and supervise clinical staff.
  • Must have experience in providing services to terminally ill patients.
  • Previous hospice management experience preferred.
  • Knowledge of hospice reimbursement requirements preferred.
  • Must have the ability to foster teamwork and lead in a professional manner.
  • Must be able to contribute as a senior level business leader of the organization.
  • Must be able to manage patient care in a caring, cost-effective manner.
  • Must demonstrate a commitment to the hospice philosophy of care.
  • Must be able to relate professionally and positively and work cooperatively with other employees at all levels.
  • Must have 24-hour access to a reliable automobile that is in compliance with all state and/or company insurance requirements.
  • Must be capable of maintaining regular attendance.
  • Must meet all local health regulations, pass post-offer drug test and pass post-employment physical exam if required.
